SUKUMVIT MEMBER CARD	SUKUMVIT FAMILY CARD
Is suitable for everyone using the Hospital?s services as there is no age limit. New Membership costs 150 Baht/person/year. Membership Renewal is 100 Baht/person/year. The card is valid for 1 year or 365 days from the day membership is paid. Benefits members will receive include: Discount of 10% for room. Discount of 10% for medicines and Lab tests. Timeframe when discounts come into effect for new members: For OPD Case Discount given IMMEDIATELY in accordance with membership benefits as stated in point 4 as soon as membership dues are paid. For IPD Case Discount given IMMEDIATLEY in accordance with membership benefits as stated in point 4 as soon as membership dues are paid. Timeframe when discounts come into effect for old members: For OPD Case Discount given IMMEDIATELY in accordance with membership benefits as stated in point 4 after membership has been renewed. For IPD Case Discount given IMMEDIATLEY in accordance with membership benefits as stated in point 4 after membership has been renewed. On expiry of membership, Sukumvit Hospital provides a 30-day period for renewal (commencing from the date of expiry). Should membership be paid later than the 30-day period, then fee to be paid will be the same as that of a new member. payment be made with a credit card together with the Member Card or other Special Cards, Sukumvit Hospital reserves the right to provide a discount of only 7%.	This card is ideal for every family that uses the Hospital?s services with no limit to age and member relationship. One membership card can be used by up to 5 persons (according to the names specified by the person who bought the membership on the ?Sukumvit Family Card Membership Application Form?. Should 5 names not be given on the ?Sukumvit Family Card Membership Application Form?, you may inform additional names to complete the number prior to the new member starts using the benefits (under the conditions of the Sukumvit Family Card that was applied for). The Hospital reserves the right not to allow change of names of the 5 members applied until the card expires, following which changes can be made at the time of renewal. Family Card Membership for the first year costs 500 Baht and Renewal is 400 Baht per card. The card is valid for 1 year or 365 days from the day membership is paid.
